It is the policy of the Union - Endicott School District to provide an environment that is conducive to the growth and development of our students. We are concerned about the health and happiness of your student. Good health and good school work go hand in hand with a cooperative effort between home and school. We will provide the best environment possible for your student to meet his/her goals.

Our school health program meets the requirements of the Regulations of the Commissioner of Education and is designed to include health services and health education. Its major objective is to promote a sound mind in a healthy body. The health program is under the direction of a supervising school physician and the Director of Pupil Services.

Immunization Information

Immunization Requirements

Over the past year, there have been a number of changes regarding school immunization requirements. Any student who does not meet the NYS immunization requirement for school attendance and does not have a medical exemption, will not be allowed to enter school at Union-Endicott. This includes access to remote learning.

For kindergarten and grades first through twelfth, the following are required:

Four doses of the polio vaccine, unless the third dose received at four years of age or older.
Two doses of varicella vaccine, except twelfth grade, which is only required to have one.
All students entering sixth through twelfth grade are required to have the Tdap vaccination.
Additionally, there are Polio, Dtap, Hepatitis B, and MMR vaccine intervals for grades pre-kindergarten through twelve.

Students entering seventh and twelfth grade are also required to be immunized against meningitis. If a child is new to New York state and entering eighth, ninth, or tenth grade without one dose, then he/she must be immunized. Please note: all vaccinations must be given at appropriate intervals per CDC recommendations. In all cases, we advise that you work with your child's provider to determine if your child has already met the vaccination requirement.

The student and their family must provide proof from a physician or clinic showing compliance with the NYS immunization requirements for school attendance.

New York State Mandated Physical Exams

New York State Education Law states that all children entering grades PreK, Kindergarten, 1, 3, 5, 7, 9, and 11, as well as all students new to the district, need to have a physical exam by a New York State physician. We recommend students to see their own medical providers when possible since your doctor knows your child best. If a physical exam cannot be provided by your own private doctor, the school physician will provide one for your child free of charge. Physical exams are done at each of the schools in the fall and spring. Please call your school's Health Office for scheduled mandated school physical days.

All physicals are good for 1 year from the date of the exam.

NY Mandated Screening: Vision, Hearing, Scoliosis

An annual vision and hearing screening will be done by the school nurse as mandated by New York State for all new entrants for the district as well as students in grades PreK, K, 1, 3, 5, 7, and 11. These tests provide only screening for potential problems. You will be notified of any deficiencies found. A follow-up examination, by your physician, will be required for diagnosis and treatment.

An annual scoliosis screening by the physical education personnel with further evaluation by the school nurse for students in grades 5 and 7 for girls and grade 9 for boys.

Dental Screening

New York State requests that students present a dental certificate for the required physical exam years; a dental health certificate should contain a report of a comprehensive dental examination.

Do You Need Health Insurance?

Are you without health insurance? Broome County Department of Social Services provides application assistance and has trained staff available to help children, teens, and adults apply for free to low-cost health insurance through Medicaid and Child Health Plus. UEHS Physical Education Excuse

According to New York State Education Regulation 135.4, “All pupils shall attend and participate in the PE program…..Individual medical certificate of limitations must indicate the area of the program in which the pupil MAY Participate.” Please take the form below to your child's physician and return it to the health office before his/her next scheduled PE day.

Medications

If a student is to take any medication during the school day, it must be in the bottle dispensed by the pharmacist with the child's name, name of medication, dosage, number of times to be given, and the physicians name on the label.

The Health Office must have a written order from a doctor as well as written parent and/ or guardian permission authorizing school personnel to administer the medication. This includes over the counter medications. Please do not send medications to school with your student. This is for the safety of all students.

Immunization Requirements

Check with your child's provider to determine if any vaccinations are needed to meet the NYS requirement for school attendance.

All students entering grades kindergarten through 4, and 6 through 10 are required to have two doses of varicella vaccination.

All students entering grades 6 through 12 are required to have a Tdap vaccination.

All students entering grades 7 through 9, and 12 are required to have a meningitis vaccination. One dose must be received after 16 years of age.

Please note: all vaccinations must be given at appropriate intervals per CDC recommendations.

Any student that does not meet NYS immunization requirements for school attendance and does not have a medical exemption, will not be allowed to enter school at Union-Endicott. The student and his/her family must provide proof from a physician or clinic showing compliance with the NYS immunization requirements for school attendance.

Illness

If you suspect your child is getting the flu, it is essential that he/she does not attend school or go anywhere else, such as childcare, the mall, or sporting events. Symptoms of the flu like illness include: fever over 100 degrees Farenheight or 37.7 degrees Celsius, cough, sore throat or runny nose. Additional symptoms may be muscle pain, fatigue and sometimes vomiting or diarrhea.

Please remember to emphasize good hand washing, cover your mouth and nose with a tissue when coughing or sneezing and dispose of used tissue in wastebasket. If you do not have a tissue, cough or sneeze into your sleeve.

Remember to keep your contacts current so we are able to reach you should your child become ill or injured at school.
